  • Can I receive healthcare services at Mosaic Health Center if I’m insured?
    Mosaic Health Center is dedicated to providing primary and preventive healthcare services to individuals who do not have insurance that covers primary care. This includes both those with no health insurance, and also those with high deductible / high co-pay plans with no way to pay these deductibles in one's current financial situation.
  • Is Mosaic Health Center a free clinic?
    Mosaic Health Center charges a modest, flat-rate fee for our services for those who can afford to pay and provides financial assistants to patients who cannot. We believe in a community-supported primary health care model, where community members who can afford to pay do so to help us keep the lights on and also help to provide free care for those who cannot afford it. Our fees are a flat-rate and completely transparent. Patients pay before receiving treatment and will never be billed after the visit.
  • Is Mosaic Health Center an urgent care center?
    Mosaic Health Center is not an urgent care or emergency care facility. We offer primary and preventive care services only, and do not maintain any physicians on-call. If you’re having a medical emergency, you should go to the nearest hospital emergency room.
  • How much do you charge?
    Patients will be charged $60 for a New Patient Appointment or Annual Physical, as well as the cost of the labs ordered. Follow Up Visits are $30. Prescription medications from our dispensary are FREE. Internal Specialty Care visits — cardiology, gynecology, dermatology, and ophthalmology— range from $30-$60. Assisted Specialty Referrals visits—mammograms, colonoscopies, and endoscopies— are FREE. We also offer referrals for free or low cost imaging and procedures, depending on financial eligibility.
  • What payment types do you take?
    We accept cash, credit, and debit payments. We do not accept checks of any kind, and we do not barter for services or offer payment plans.
  • What do I bring to my appointment?
    Please bring a form of ID including a driver’s license, passport, or school ID. If you’re a new patient, you can also fill out and bring our new patient forms here.
  • What if I need to cancel my appointment?
    If you need to cancel your appointment, please provide us 24-hour notice by calling or texting (678) 383-1383.
  • What if I miss my appointment?
    If you miss your appointment, you will be charged a $20 missed appointment fee at the time of your next visit.
  • Can I see a provider if I don’t have an appointment?
    We encourage our patients to schedule appointments in advance by calling or texting (678) 383-1383. We offer a limited number of walk-in appointments daily only if our schedule permits. You may experience a wait or might not be able to be seen at all if you arrive without an appointment.
  • Is every provider at Mosaic Health Center a medical doctor?
    Our staff includes full time and volunteer medical providers, including physicians, physician assistants, and nurse practitioners. Patients are assigned to providers on a random basis; however, a licensed physician supervises all providers.
  • What if English isn’t my primary language?
    English is not the primary language of many of our patients. Our diverse staff reflects the community that we serve, with employees from over 8 different countries and speaking over 13 languages. Our exam rooms have interpretation capabilities, and we’re able to communicate with patients in over 300 different languages.
  • How do I get prescriptions and prescription refills?
    Mosaic Health Center has an on-site dispensary where patients can get their prescriptions filled right after their appointment. All prescriptions filled at our dispensary are FREE. If you need a refill, you can make a follow up appointment with us by calling or texting (678) 383-1383.
  • What if I need referrals to providers that are outside of Mosaic Health Center?
    Mosaic Health Center offers FREE Assisted Specialty Referral visits for mammograms, colonoscopies, and endoscopies. We also offer referrals for free or low cost imaging and procedures, depending on financial eligibility. Our Wellness Team provides community health education and navigation services to help support you in your needs. If necessary, we have agreements with other specialized providers and will refer you to those providers. Through our patient navigation services, we will assist you in navigating care for any healthcare services outside of our network so you will continue to receive our support for coordinated care.
  • What are the hours of operation and location?
