ELIGIBILITY
Mosaic Health Center is a community supported health clinic for uninsured individuals. We only serve patients who are age 18 or older and have no health insurance.

Please download, print, and fill out the following new patient forms and bring them to your first visit.
If you are unable to print the forms, we are happy to provide them to you at your visit. Please arrive 10 minutes early to allow time to complete the forms before your appointment.
COST AND PAYMENT
In order to provide high-quality primary and preventive healthcare for uninsured individuals in our community, Mosaic Health Center charges a modest, flat-rate fee for our services. Services are a flat rate and completely transparent. Patients pay before receiving treatment, and we will never bill after the visit.

ASSISTANCE
Financial assistance is available to patients who cannot afford to pay fees, and Mosaic Health Center will never turn away anyone uninsured from receiving primary and preventive healthcare who can document financial need. Please speak with our front desk staff for details.

Our diverse staff reflects the community that we serve, with employees from over 8 different countries and speaking over 13 languages. Our exam rooms have interpretation capabilities, and we’re able to communicate with patients in over 300 different languages. No matter your language or where you're from, Mosaic Health Center is committed to creating positive outcomes and helping those without insurance to navigate the oftentimes confusing healthcare system.
