Lifesaving Services For A Diabetic Patient

At Mosaic, we believe good health is more than an annual checkup-- it's giving people the knowledge and support needed to be healthy.  For many patients, understanding chronic illnesses or healthy lifestyle behaviors is a brand new concept.  Through our Wellness Program, patients receive health education, food access, regular follow-ups, health monitoring, and encouragement to help them take control of their health.

A 51-year-old Hispanic gentleman came to Mosaic with an A1C (blood glucose test) of 14.0.  His high reading put him at risk for diabetes-related health complications.  Though hesitant to join our Wellness program at first, he is now successfully managing his diabetes and high blood pressure through medication and healthy lifestyle changes.

Mosaic was able to provide free insulin at our pharmacy so he never misses a dose due to cost, as well as diabetes management education.  Combined with ongoing phone calls from our team supporting him, he saw significant results-- dropping his A1C to 7.7 within three months.  As of a year later, his most recent reading was 6.8, which is below the recommended A1C for diabetes.  We're so proud of his commitment and our team for helping him achieve a healthy future!
